Kidney Failure

Question
We have an eleven year old shepherd wolf mix named Jonah who we love dearly. After recent blood and urine work the vet has deemed his kidneys at 25% effective and the vet recommended a low protein diet. In addition, his teeth are in awful shape and the surgery could kill him. I want him to be as healthy as possible and do the best I can to keep him alive and happy for as long as possible. We feed him a combination of venison dog food and recently have been mixing it with a home made mixture of turkey/brown rice/veggies. He is a picky eater and really likes the home made food and since has gained 10 pounds. This is a good thing as he has always been underweight due to unknown allergies altho we suspect beef. What do we do now? What is the recommended diet for our picky boy? I have spent hours on line to find out an answer and can't find any recipes or answers. We are happy to make his food. Can you help with any recommendations?

Also, some Vitamin C - ESTER C - 500 mg per day is great for dogs with Kidney stones and Kidney failure.
Ester C can be found just about at any pharmacy...
