I have a miniature poodle that is about 1 year old.  We got her about 3 months ago and she was great for about 2 months.  She never had an accident in the house and she doesn't have an accident in the area we keep her while we're gone (about 8 hours).  But we will take her out multiple times in the evenings, she will go while outside, but once returning to the house will have an accident shortly after coming inside (sometimes within 15 - 20 minutes).  We stay out a long time and take her out frequently, but it doesn't seem to help.  What can I do?

Start all over at square one and absolutely do not give her full run of the house until she has proved to you once again that she is completely house broken.  If she is going that quickly after coming inside, even though you are out there a while she isnt paying attention to what needs to be done and isnt finishing.  Let her go potty, take her in a for a few minutes and keep a close eye on her and then take her back out.  Praise her highly when she goes out and give her a treat after she is done going.  Give her a strong NO when she does it in the house and take her immediately outside.  Remember once again, DO NOT GIVE HER FULL RUN OF THE HOUSE.  That is the #1 mistake made.  Make her prove herself.
