My friend has a Jack Russell and she constantly itches raw under her arms pits and ears.  My friend has taken the dog to a vet before and the dog gets a shot for relief of itching but it does not really help.

Right now we are using Bag Balm(cream)for cow utters, and Gold Bond for itching and rashes.

She also wears a harness and collar and I think that is irritating her.  

What do you think about food?  She eats dry lamb and rice.

What do you recommend for treatment and food?

This sounds like allergic dermatitis.  There is a shot to help stop itching, but it does not last long.  We follow the shot with tablets that you would give at home to keep the skin from turning red and itching so bad.  Your veterinarian will be able to help you with this.  If there is a skin infection in addition to the allergic skin, that needs treatment also.
