AREA IN QUESTION
I RECENTLY BOUGHT A MONITOR (COULD BE WATER OR NILE)FROM A YARD SALE THE PET STORE SAID HE IS A VERY NICE LIZARD BUT HE HAS A SACK HANGING FROM THE BOTTOM OF HIS TAIL IN BETWEEN HIS BACK LEGS.ITS RED AND NOT GOING BACK IN.ITS BEEN A WEEK AND IT DOESN'T LOOK ANY BETTER WE HAVE LEARNED TO LOVE HIM AND DON'T WANT TO LOSE HIM. HE'S ABOUT 1 1/2 FEET LONG AND LOOKS HEALTHY EXCEPT FOR THIS AREA I'M WRITING YOU ABOUT.THE CLOSEST REPTILE HOSPITAL IS TWO COUNTIES AWAY.CAN YOU HELP US ?

OH poor thing!
That is a prolapse.  This is usually caused by improper husbandry.  I cannot quite tell from the picture if it is a hemipenal prolapse or a colon or rectal prolapse, but any of them are not good.
For now, make up a cool sugar water bath for him immediately.  Mix 1/2 cup of water & dissolve it into some warm water, then, add cool water & soak him in it for at least 20 minutes.  This can "sometimes" help to reduce swelling so the area can retract back in.  However, it is no guarantee that this will be successful.  
In the meantime, make sure he is on a non particulate substrate such as paper towels so the area doesn't get dirty.  Also, get some vaseline or KY jelly to generously lubricate the area to help keep it moist.

I am sorry, a lot of times this does require vet care if the area does not retract back in on its' own.  
Are you using a good UVB light for him?  What are the temperatures he is being kept under?
How often are you using calcium?  

Let me know how he is doing.  Treat this as an emergency, so if it doesn't fix itself soon, he will need to see a vet because the area will become necrotic & amputation & surgery will have to be done.
