Question
We have a guinea pig about 7 months old.  He has these scabs
around his neck area, by his ear, as well as on his back.  They look
like really sore scabs.  They are kind of bloody.  When he scratches
or something he makes a high pitched sound like it hurts.  He has
been eating well and receives good care.  We are from a small town
and I'm not sure if the vets have ever dealt with guinea pigs.

It sounds like your guinea pig has mites. It could be a pretty advanced case if he is indicating pain by squealing, and has numerous sores on his body.

Mites cause deeply intense irritation and even seizures if you don't get them treated.

It is worth contacting your local vet to ask how experienced they are with treating guinea pigs. You will need to visit the vet at some time, and if you can then this is one time when you could really use some professional advice from someone who can see the pig for themselves.
