Is it possible for a 7+ year old Shih Tzu to get pregnant, and if so what are the health risks to her?

As long as she goes into a normal heat then yes she can get pregnant. The risks are the same as with an older women. At 7 years old that is essentially the equivalent to a 50 year old women getting pregnant. There is a higher risk of deformities, absorption of the litter and still birth. I would suggest that you get her spayed, at her age I would recommend that you do a blood panel to make sure that everything is good with her internal organs. At this point she is still susceptible to pyometra and ovarian cancer but it will still benefit her to NOT go into heat for the next 5-7 years of her life.
