Travel with Pet

We are leaving in Oct 2008 and flying from LAX to Frankfurt, Germany. We have a jack russell terrier who has had her rabies shot in July 2008 and will be receiving the "5 in 1" shots this evening. What other documentation do i need? where do i get the appropriate paperwork? who fills it in? and she is microchipped as of 2007.

That is way outside my area of expertise.

I would speak to the airlines about what the requirements are for international pet travel.  Then I suspect you'll need some kind of paperwork from your vet... but the airline will be able to tell you the forms you need.
