Housebreaking

Question
My 5 month old Maltese came to me several months ago pee pad trained.  We
have been working to train him to go outside.  For the most part, he does
quite well.  But at least once or twice a day he pees/poops inside.  Often, this
is just after we have come back INSIDE.  I take him out frequently and reward
him for doing his business outside.  He NEVER pees/poops in his kennel
(even overnight) so obviously he has the capacity to hold it.  What do I do to
stop the peeing/pooping inside?

Is he going in the same places in your home>?
If so i would suggest stain and odour remover as the previos scent may be there.
Other than that constant praise and persistance will prevail.
Maybe wait with him outside till he has "gone" and again keep regular intervals in going out.
