Bring 2 cats from US to Germany, then onto Sweden

Question
What are the requirements for bringing two cats from the US to Germany for a while then possibly moving onto Sweden?  I believe the first step is putting microchips in them - which I am doing this week.  What is after that?  Where do I find the official forms needed?

Dear Tammy
Easy to Germany.  Microchip.  Rabies vac at least 30 days old and
less than 1 yr.  APHIS 7001 International Health Cert issued within
10 days of travel and endorsed by usda.  Also, EU form 998
bi-lingual English-German recording microchip and rabies info.  Also
must be endorsed by usda.
Sweden a bit more involved. If going straight to Sweden from USA
would require above plus rabies titre blood test and a wait of I
think 3 mos.  Best for you to contact Swedish authorities.  Contact
Swedish Embassy or consulate for address in Sweden.  Spending time
in Germany may reduce this.  As soon as you get to Germany, contact
a local veterinarian and proceed to get EU pet passport.
Sweden should not be quite as difficult as UK
You can go to p. 4 of my:  www.actionpetexpress.com  and contact
DEFRA for any up to date info.  These requirements can change from
time to time.  Also, visit:  www.ipata.com  There are pet shippers
in Sweden who can also update you.
