Run out and get some milk replacer. My favorite is Just Born Milk Replacer. Get the liquid as you shouldn't need it for too very long, and the powder would last longer than you need it.

Mix the milk with some baby rice cereal or preferably some ground-up puppy food. Start out as a thin gruel and gradually thicken it more each day with more solid food, feeding four times a day at this age. Always have water available as the puppy will start drinking on its own soon.

Depending on how well the puppy is eating, in 1-2 weeks you should be able to put it totally on kibble, by soaking the kibble with water until it is softened enough for the puppy to eat. As the days go by, and the teeth come in, you should be able to put less water in the food until the puppy is eventually eating it totally dry.
