Beta fish:Vertical

Our beta fish (about 1 year old): Standing vertical, listless, breathing heavily (especially near the surface).

Have you had him for a year or you know he is a year old? (They are usually almost a year old when you buy them)
What size tank do you have him in?
How often do you clean the tank?
Do you use a water conditioner when you change his water?
Are there other fish with him?
Does his belly look bloated?
