veterinary dialogue

Hi Ellen,
I don't have a question specifically about a pet, but was hoping you could help me out. I teach english in Poland and have a student who is a veterinarian and would like a sample dialogue of things vets ask in english, as she sometimes has foreigners who come to her. If it's not too much of a hassle could you email me a sample dialogue or commonly asked questions. I've been searching the internet with little luck and resources like this are not easy to find in Poland. And I've never had my own pet(sadly) so I have no first hand knowledge.

specifically we ask this:

What do you currently feed your pet?
Any ABNORMAL vomiting, diarrhrea, coughing or sneezing?
how are the bathroom habits?

those are just the basics. now if the pet is having a problem, we get alot more specific.
