Little white dots on the fins could be the first sign of a disease called ich.
Absolutely you need a filter. Even with a filter, you should be changing 25% of the water once a week, every week. Changing less water more often is far better than changing all of the water less often.

There are many medicines out there for ick. I use one called Rid-Ick. Quik-Cure is another one. Whichever one you buy, follow the directions carefully.
You leave the cichlid in the tank.
