R.E.S. in trouble

Question
I have a 26 year old R.E.S.  He has been tanked inside for 26 years at my house without any major health problems.  Now for some reason, he has basically completely stopped using his left front arm,  and also has a bubble in his skin, right near his shell on top of his neck.  The bubble is completely clear, and i have never seen anything like this before, since he is the only turtle I've had.

At 26, it is a senior citizen and we would expect to see health issues starting up. As my bio mentions, I am not a vet, and I have not heard of this sort of thing either.
