Hi Kim, got a couple of questions to ask. I would like to find out if my mini
poodle, which is just two weeks short of a year old, should change his dry
knibbles to adult formula when he turns a year old. He is now currently on
EaglePack Puppy Formula. Secondly, recently (for a few weeks now) when I
give him cooked chicken, he would throw up immediately. However, when it
comes to other food like carrots or apples, he is fine. Can I know why he is
throwing up the cooked chicken when he has not done this before? He has
been on a weekly cooked chicken diet for 4 months now.

Hello Mabel,
Miniature Poodles tend to mature slower than the Toy Poodles so if you wanted to continue feeding the Puppy formula for a few more months thats ok.  It would still be good for him and if he needs it, it would help fill him out some.

If the chicken is making him throw-up, I would stop feeding it to him for now and switch to turkey or lamb.  Go slow with the switch so as not to upset his stomach, I would say give him about 2 tablespoons with his food, then increase it until he is getting about 1/3-1/2 cup or so (or whatever he is used to getting)
I can't think what would be causing him to throw-up the chicken, unless he is suddenly reacting to something in it.  
Go ahead and give him the apples and carrots as they are healthy for dogs and if he eats them, good for him.  My Poodle would eat as many baby carrots as I would feed him, he Loves them.
