Cat scratching her fur off

I took my cat to the vet five days ago because she is scratching like crazy and she is tearing her fur out.  The vet prescribed Revolution.  She is even worse now than she was several days ago.  I've called the vet, but they haven't returned my call.  I don't know what to do next.  She looks so uncomfortable.
