found small hard knot below his penis and he is whining and crying when he moves. He has been neutered about 5 yrs. ago when we got him.

I'd take him to the vet. There are several things that it could be, Like an injury, a deep bruise or an infection. So please, do have him vetted asap. Give him some infant's tylenol (does by weight according to the bottle) while you're waiting for your appointment.
