Cloudy Urine & Mucus in Stool

Hello,
I have a five week old pitbull puppy. I know it is too young  to have, but the mother will have nothing to do with him. Anyway, I've recently noticed that his urine has been cloudy. As far as what he's eating, I'm giving him the puppy mil replacement formula mixed with dry kibble. I just recently had hid de-wormed, and now my husband has noticed mucus with a bit of blood in it, in the stool. I know you are not a vet, but I need some advice as to whether what I should do, is it serious enough to take him to the vet?

Your puppy needs to be examined by a veterinarian, and a stool sample needs to be taken in and analyzed. He could have worms or coccidiosis or giardia and those would need to be cleared up with the appropriate medications. The vet might also want a urine sample as the puppy could have a bladder or some sort of bacterial infection.
