My Cat is getting thinner...

Hello, my name is Chris and i have a question about my two year old cat. He is a male and spends a good deal of time outside... He eats normally, every couple of hours and he looks ok except that he literally looks like he is starving! I don't know what to do. I have given him worm medicine and am hoping that this will help.
