sick beta fish, swimming vertically - help?

My beta fish has always hovered near the bottom of his small tank
which I was told was okay.  I keep the tank clean and feed him
everyday as instructed.  Lately, he has been laying on his side on
the bottom of the tank and not moving much at all.  Please, what
do you think it is and what can I do to help him? EXTREMELY
WORRIED.
