Unable to potty train

Question
My 11 month old puppy will potty (both) in her wire cage and in the house. The longest she is left in the cage is over night. We let her out all the time and she is rewarded with treats when she goes outside!!!

Try feeding her in her crate (dogs don't like to poo where they eat), or don't use a crate, and, instead, use a waist leash (buddy leash) and tether her to you so she can't go hide her "accidents".  This is the method I use:
