Potty training troubles

I have a 6mth old shihapoo. I have religiously tried to potty train this dog and nothing is working. I let her out at 7am then feed her at 7:30am a 1/2 hour later I let her out again. After that anytime I see her sniffing I let her out. When I have her outside on the leash or off I use the command "go potty outside" after she makes I praise her immediately and bring her in and give her a treat. After the treat I keep her in the crate for 45 minutes so she remembers the last training we did. The problem I'm having is that when I do bring her outside she doesn't always go she'll look around for about an hour. Sometimes she will go outside and then when I bring her in she makes again on my carpet. Sometimes she will just poop and pee in her crate. She even pooped on my bed without giving us any signs that she had to go. Usually I can tell when she can go, but sometimes she give no indication and just goes wherever she feels like it. People say just to crate her, but if I do that she'll just make in that as well. Whiel in the crate she just barks continiously. She's also nipping at my babies feet as well as my older daughters feet. I know you need to tell her a firm no and give her a chew toy, but she doesn't seem to be getting it. I'm so frustrated at this point that I'm ready to give her away. Can you give me any advice. I hate to break my kids heart, but at the same time this is frustrating for the dog as well as myself.
