sometimes when my cat is in my lap, while rubbing him,     I feel his ears and they feel hot. (He loves for me to rub his head and ears, and under his chin), other times
the fleshy part of his paws feel very cold. Is this a sign of being sick?

No, hot ears are not necessary the sign of a sick cat.  The skin and fur about the ears is very thin and the normal body temperature for a cat is about 100.5 - 102.5 degrees (to our 98.6), so this is some of the explanation they may feel hot.

If your cat were not feeling well, you would notice behavioral changes, usually lack of appetite, general malaise and the like.
