How to help a rescued dog

Hello Kristin; I just adopted a 6 year old Maltese. He was rescued from a Puppy mill and is afraid of contact with people. He will follow me around but always keeping a distance, same thing if he chooses to lay near me (if I'm sitting down by the computer or watching TV) it is always within a 3 to 4 ft. distance.
If I lay on the floor next to him he will lay as close as 2 ft away and close his eyes, but any movement or unexpected noise will make him open his eyes right away and get nervous.
He will not take anything from my hand. Apparently he is terrified of hands. I have not been able to "catch" him so that I could take him out for a walk. The first day I brought him to my house I noticed that he would not walk with a leash, he basically becomes still and won't take a step. I don't chase him at all, I let him be and let him be the one who comes to me if he feels comfortable.
I need to take him to the vet tomorrow and I have no idea how to catch him. He knows the house pretty well and is extremely smart. So it makes it impossible for me to "trick him" so that he comes to me, he is way ahead of the game and, apparently has been tricked too many times to trust anyone.
If you have a suggestion that can help me I will appreciate it. I thank you in advance for you advice.

What you could try is to buy a crate for him,and leave it open,and hide(not to far away and in a small room)and when he goes in it,shut the door. This way you won't have to scare him anymore than he already is,they usually go in a crate on there own so chasing isn't usually necessary,it would also be a good idea to leave it in the room on it's own for a few hours to get him used to going in and out without being trapped
