shots

Question
I ahve a cute rat terrior named Buddy.I bought him in Sept.from a gal 30 miles form where I live.He was born on July 14th I was told (no papers yet to verify that).So I figure he is 4 months old.She said he was wormed when he was a few weeks old.I went to local home of economy store and purchase his first shots and gave them to him in Oct.My Question is Does He need a second set of shots and what about being wormed do you repeat this?Do you have a copy of a shot record that you copuld e-mail me so that I know when and what he needs.
