Answer
Hello Detra,
What you have to determine with your dog is; What is her quality of life now?  Is she still healthy enough to go on, is she in any pain, and does she look like she is able to comfortably continue living?  If she is well enough to continue living, there are measures that you can take to help her live as comfortable life as she deserves.  You can buy Potty Pads, or even Doggie Diapers, so that she doesn't potty on your floor.  You can do things like using sound to help guide her, soft blankets or bedding to ease her joints and muscles, or buy special foods for older dogs.
If she is not doing well and you see that her health and attitude about life is poor, then putting her to sleep might be the best thing for her.  You can arrange a time with your Vet to take her in and you can either stay with her while they put her to sleep, or they can handle it for you.
Toy Poodles tend to live for around 15 years plus, so she is reaching the end of her years.  Her body is starting to shut down somewhat, which is why she is not hearing well, seeing well and is starting to potty in the house.

Find a friend that can go with you or be with you when you make your decision.  It will help.

I hope this helps you some.  Please feel free to ask other questions about this, or other Poodle questions in general.
