Hi,
I have a four year old female cat.  She has white long hair and she is very large but not overweight.  She was spayed when she was about two or three months old.  Now she weights about 18 pounds and is almost twice as long as my other cat. I started noticing that she had a urine smell and she was very dirty around her anus and vagina.  I took her to the vet and he said it was a yeast infection.  He prescribed miconazole nitrate 1%.  I have been using it for the prescribed amount of time, two weeks.  But her condition has not improved.  One thing worth noting is that because of her size and long hair she has always had a hard time keeping herself clean specially her back end. This is why I take her to the groomers about twice a year where she gets a bath and a hair trim.  Another thing is that right before I noticed her condition I switched her food from Eukanuba to Inova evo and about that time she developed a urine smell.  I don't know if that was the cause but it is worth to mention.  After a while I had to switch the Inova evo because it gave my other cat allergies (chin acne).  So now they eat Wellness indoor dry food formula and Wellness can cat food.  She seems happier now that she is eating Wellness but no improvement in her condition.  What do you recommend I do?  I made an appointment with the vet for next Tuesday, but I feel I need to be more informed about the condition and treatments available.  Unfortunately, I haven't found much on the internet. Sorry this is long and thank you for taking the time to help me and my kitty.

Remember, I am not a vet!  I have no idea, but let me hazard a guess.  If she is dirty around her genitalia and anus, I suspect she has a medical problem.  It may also be what you are smelling is not urine, but musk./  She may even have impacted anal glands.

Yes, a visit to the vet is a very, very good idea.
