Answer
I am going to tell you to put it on a strict potty training schedule  starting completely over as if it were a puppy. You cannot wup a dog for any reason. If your dog potties in the house, you HAVE to interrupt it AT THAT TIME, you cannot do it after the fact and you should never scold or hit. You cannot get reprimand a dog for ANY misbehaving unless you CATCH IT IN THE ACT.

HOUSEBREAKING TREATMENT PLAN
Beginning at 8 wks old to 6 months the puppy will stay in a penned area near the door they are taken out to go potty. Their crate will be in the pen with the door open and a chew toy and small amount of water will be inside. Potty training pads will be placed as close to the door as possible. As the dog gets older gradually increase the size of the pen as long as they behave until about 6 1/2 months of age when you can try letting him have the entire room as long as he does not potty inside.
When the puppy is 4-6 months of age he won't need to go out as often as when he was little.
When he is little he should be taken out about every 30 minutes. Free times should be on leash supervised free times not to exceed 30 minutes at first until at least 6 months when you can let him start dragging the leash around without you holding it but keep an eye on him and if there are accidents he immediately loses that freedom. By 8 months he can be let loose around with the family while he is calm in the house and should still be let out to relieve himself about every 3 hours just for comfort reasons so he remains calm.
Outline of freetime, potty time and feeding times for 8 wks -4 months
6 am: pup wakes up and should immediately be taken outside
615-630am: if pup goes give 10-30 min on leash supervised free time.
630-635am:take pup out again, bring pup back in after 5 min whether or not he goes, if he didn't go feed him in crate or watch him closely
635-645am:Feed puppy, make him wait for release word before he starts eating. Give 10-20 min for meal making sure pup doesn't wander off. Take pup out immediately after finishing eating.
645-7:00a.m: take puppy out, he will probably go, if he does praise him and bring him in for 15-30 min on leash free time. Make sure an extra 10 min is allowed outside after eating if pup goes in first 5 min.
7-730am: you can allow pup to follow you around on leash to investigate and learn how to behave indoors. Playing should be done in the yard or else go for a walk.
730-745am: confine in crate or tether pup while getting ready for the day since free time is over.
745-8am: take pup out one last time before leaving and then put him in his penned area.
8AM-4PM OWNERS GONE
4-415pm: take pup out
415-445pm: pup gets 30 min free time if he potties otherwise put in crate during this time and try again in 15 min.
445-505pm: leash pup, bring inside and give food and water
505-610pm: take pup outside and leave him in the yard to play, before taking him in take him to potty area to relieve himself.
610-640pm:30min free time if pup goes
640-645pm:take pup out, he probably won't go so bring him in and give him some more water to drink
645-7pm:bring inside and put in crate or tether him
7-705pm:take pup out, he should go this time
705-800pm:Bring him in and tether him somewhere in the room with the family. Give him chew toys to play with and encourage him with praise for playing with them. Owners should spend 20-30 min playing with the puppy and his chew toys for encouragement.
800pm: take puppy out to potty, if he doesn't go bring him in and confine him for 15 min and then try again.
OWNERS SHOULD CONTINUE WITH THIS SCHEDULE OF GOIN OUT EVERY 30 MIN FOR 10-15 MIN WITH A 30 MIN FREETIME FOR POTTYING AND A 15 MIN CONFINEMENT FOR NOT UNTIL BEDTIME.AT BEDTIME,THEY SHOULD TAKE HIM OUT TO POTTY ONE LAST TIME AND THEN PUT HIM TO BED IN HIS CRATE FOR THE NIGHT. IF THE PUP SHOULD WAKE DURING THE NIGHT TAKE HIM OUT TO POTTY AND IMMEDIATELY RETURN HIM TO BED.
This schedule should be followed every day for at least the first 4 months at which point the pup won't need to go out every 30 min.
Do not punish for accidents, if you catch him peeing try and stop him so he finishes outside and then clean up the mess when he is not around.
