4 year old cat defecates next to litter box- Could it be pancreatitis? Please help!

My 4 year old cat will urinate in the litter box, but defecates right next to it.  She did this once before in a different house, but then all of last year was fine.  Now we live with another cat and have three litter boxes, two in the "kitty area" and another I've been keeping in my room.  The dust is causing me severe chest pains but I don't know what else to do because my roommates are frustrated.  She recently vomited a lot and I took her to the animal hospital.  She was there for a week- they ran ultrasounds and Xrays and found nothing.  They eventually put her on anti-nausea medication and sent her home, and told me to give her 1/2 a Pepcid AC along with the medicine.  She is eating fine but still defecating the same way.  Could this be pancreatitis?  I am running out of ideas and also my chest is in a lot of pain.  Her stool looks normally formed but one peice did look a little soft.

The signs of pancreatitis are vomiting, lethargy, and tenderness around the abdomen. The animal usually vomits up green bile which is coming from the gall bladder. That is another sign that it is possibly pancreatitis. Diet is something here to be considered but since you didn't tell me how old she is or what you are feeding I cannot comment on that.

Defecation outside of a litter box when the cat is urinating in it is more of a sign of displeasure and inappropriate elimination.
One cause of this can be where the litter boxes are- and too much traffic around them. She is telling you that she doesn't like the place that they are, or the litter in them. If you changed litter that will make them not use the box. Putting the box on some aluminum foil (hanging out from under it) will sometimes keep them from using the area around the box, as will putting a big piece of plastic under it.

First of all you need to find out what is bother her inside.Did she have blood work done? It is hard to find pancreatitis on an xray. You need a blood panel to determine if she is having a problem with her pancreas.
Her liver enzymes will be elevated.

You need to remove the box from your room if it is affecting you like that.

I hope you can figure this out before you develop asthma and she has more problems with her liver/pancreas.
