Answer
Hi Tooker,
This is a disease called hole-in-the-head. They usually affect fish such as cichlids, Discus and oscars. It is usually very difficult to treat and the cause in unknown.
Because there may be multiple causes of this disease, the treatment usually consists of taking a multi-faceted approach. The goal is to rid the fish of Hexamita, improve water quality, and improve vitamin/mineral supplementation and nutrition. Use an antibiotic, metronidazole to treat the fish. Meanwhile, daily water changes are a must.

Good luck!
Chloe.
