Hi, I've had my betta fish for 1 year.  I had her in a large fish bowl using tap water, sitting it out for 2 days before placing him in there.  I saw a betta kit at the store and bought it.  It came with a fake plant, gravel, food, and conditioner.  I fed her freeze dried tube worms every day before changing her tank.  2 days ago I used poland spring natural water, and put half a capful of the water conditioner in there. (thats what is said on directions).  Washed the gravel and plant off.  I also had some shells that I used in the older tank with him from the beach. (small ones).  My boyfriend added a larger shell from the beach the day we switched her tank, but he washed it off.  The day I put him in his new tank, he seemed fine, not swimming a lot, but he didnt eat.  Now today, 2 days later, he was floating at the top on his side, kind of curved his body. Now a few hours later, he is at the bottom digging his face in the gravel and shells.  I cooked a pea and tried to feed it to him, and a little piece floated to the bottom next to his mouth, but he wont eat it.  A few hours ago he was at the top sideways, but now is at the bottom just sitting in one spot, with his tail in the air.  At times he will move around.  I don't know what's happening.  I changed the water before and added fresh poland spring and added a little bit of salt in the water, because I read that can help with the oxygen.  I only added a little.  He still won't eat anything, and 2 days ago when I originally switched him in the new tank, I tried giving him the flake food the kit came with, and a little tube worm but he didnt eat it.  Here is a picture of what he looks like right now.

 Sorry to hear your Betta isn't well. I know how that is with 12 adults of my own. May I suggest taking him out of the Betta kit. They are too small for a full grown Betta male. They need to be in nothing less than a 2.5 gallon tank. They also need a heater. They are tropical fish needing heat around 82 degrees. DO NOT use spring water. They add a lot of sodium and other things that are not good for fish, especially delicate fish like the Betta. You cannot just wash shells that come from the beach. You need to boiling them for 15 minutes, then let them cool off before you put them in his tank. What kind of conditioner are you using? I would remove everything from his tank but him and his heater. Use regular tap water with the conditioner. Turn his heater up to 84-86 degrees. Since adding heat depletes the oxygen, add a air stone to his tank as well. The air stone adds the oxygen back into the water. Do 25% water changes everyday for a couple of days. Make sure you check he water perimeters every week. I have 12 adult Betta's in their own tanks and i check their water every week. Whatever food he doesn't eat, make sure you remove it from his tank.  I am thinking that when you do all this, he should start bouncing back. I don't think he has a disease. Sometimes moving him from one house to another can cause him to stress. Betta's can even get depressed, especially if they don't have enough room. Try putting a mirror up to his tank for a few minutes a day and see if he react to it. Let me know if after you try all these and he isn't better.
