Cocker spaniel inappropriate urinating

My boyfriend has a male cocker spaniel that is one year and three months old.  Lately he has been urinating in the house, mostly in front of me (sometimes excited, other times not excited).  At one time, he pooped at my feet while I was standing in the closet.  What are the possible reasons for this behavior and ways to correct the problem?

How long has this been happening?
Who walks him?  How often & for how long?
Who feeds him?
Where does he sleep?
Is he neutered?
Was he perfectly housebroken previous to this?
