Ponazuril and anti-inflammatory meds to treat e.cuniculi ?

22 10:54:49

Question
My 6 1/2 year old lop has e.cuniculi, and even though she didn't have symptoms, she was treated with the fenbendazole five years ago, which did bring her titer levels way down.  But recently weakness in her back legs seemed to be symptoms.  (she also has a crooked spine, but the x-rays didn't show a decline, but without an mri to tell for sure, it could be a problem with her spine) SO, the vet gave both anti-inflammatory meds and the new drug Ponazuril.   It's been about 12 hours, and I am shocked to see a little bit more strength return.  Can that be possible?  It's not a big change, I mean she still can only wash her face with one front paw at a time since she can't lean on her back legs, but she's more sturdy when she hops and she stands up straighter.  Could this mean that this is her spine, not e.cuniculi? Also, I read somewhere that anti-inflammatory drug is sometimes given with Ponazuril.  Does that mean that e.cuniculi can cause a flare up in the spine?
Sorry, I know lots of question, but it's sort of a labyrinth of possibilities to one symtpom.

Answer
Dear Jen,

Since you added *two* variables to the treatment instead of just one, it's not (yet) possible to know whether the improvement is due to the anti-inflammatory drug or to the Ponazuril.  My guess would be the former.  But in my experience, Ponazuril works *incredibly* fast to kill microsporidians in the intestine.  Who's to say it might not have a fast effect on those in the central nervous system?

I'd be very surprised if the drug worked *that* fast on the parasites, though.  But who knows?  Perhaps the combination of Ponazuril (to kill the parasites) and anti-inflammatory drug (to reduce/prevent inflammation resulting from lysis of the dead parasite cells) could have a "one-two punch" effect.  I think your vet had a good idea by combining the two!

Hang in there and hope for the best.  Ponazuril is an amazing drug, and I'm surprised more people are not already using it for E. cuniculi.  I hope it works for your little buddy, and that he's up and running and back to his normal self soon!

Take care,
Dana
