One of my foster cats is maybe pregnant, but it's hard to tell. Supposedly she's been spayed. She's pretty round, but she's also fat, so it's hard to tell why she's round. Her nipples, however, are not pink or swollen or anything, just normal, small, light nipples. Is there any chance she is pregnant and her nipples simply haven't swollen yet? Everything I've read says that's the first visible sign, so I was wondering if there are cases where it doesn't happen?

If your cat is in fact spayed the fatness could be a case of 'good living'.  However, assuming she may not be, the first sign of pregnancy is at around 21 days and the nipples get pink and enlarged.  As the pregnancy advances, she will lose some hair from around the nipples until around 6/7 weeks you can then see the kittens moving.  From what you say her nipples seem normal and not a sign of pregnancy so it may well be that she likes her food.  I certainly have not had a pregnant cat that does not have enlarged nipples.  If she is normal in every way, eating, drinking and playing, i.e. not sitting in a depressed state as if not feeling well, I would say that she is just making the most of the good food you are giving her.  You may need to think about cutting back a bit if this is the case.
