I have a twelve year old Labrador.  Recently, his testicles seem to have to become enlarged, and appear to be sagging.  Is this anything to worry about?

I don't think you should worry, I do think you should be concerned. Take your dog to the vet. Most likely what he has are testicular tumors. These are usually benign but can cause all kinds of symptoms and problems. Though they are usually benign they are not something that should be allowed to continue without treatment. The good news is that they rarely spread and rarely become cancerous. But again. I cannot urge you  strongly enough to go ahead and get your boy to the vet ASAP.  Not an emergency situation but it is something that doesn't need to be put off. There are types of slow growing malignant tumors , usually found in the abdominal cavity that can also cause these symptoms. There are other possibilities but this would be the most likely. Of course I am only guessing and without a hands on evaluation of your dog I couldn't say for sure. This isn't a diagnosis, just an opinion. Please do let me know what your vet discovers
