Answer
You need to take a stool specimen in to your vet to determine if he has worms or not.

At four weeks he can be started on a gruel of baby rice cereal and ground-up kibble with some milk replacement mixed in. It should be a bit on the thin side at first. Gradually make it a bit thicker and mix in more ground kibble each day. Eventually he should be switched over to kibble soaked in warm water, and as the days go by, less and less water is added until he is eating it totally dry by around eight weeks. Start out feeding him three times a day, and then take him down to twice a day by around eight weeks.

Always have water available for him during the day.
