My russian tortoises shell on her underbelly is missing a piece I do not know what happened, what would cause this.

A scute (scale) is missing? This can be due to injury or shell rot. If the tissue under the missing scute is pitted, discolored, soft, or otherwise abnormal, it could be a sign of one of the many forms of shell rot.

There is a hole or opening in the shell into the tortoise? This is usually due to an injury.
