hi, i'll be adopting a mid-sized dog soon. i live in the us but i'm from poland originally so i visit my family couple of times a year. i want to take my dog with me and i'd like to know how to prepare my pet and myself for the long trip to poland and then back to the states. also, what formal steps will i have to take so there are no problems at either border (quarantine, shots, certifications, passports etc.). thank you

To enter Poland (most all EU Countries) from USA, dog will need:
microchip,  rabies vac at least 30 days old and less than 1 yr.
APHIS 7001 international health cert issued within 10 days of flight
and endorsed by usda.  Also, most important:  EU form 998  English-
Polish and also endorsed by usda.
You don't tell me where you are?  What airline do you fly on?
If you take any flight other than one thru UK, you should be able to
take dog as excess baggage.  Much cheaper, and would clear customs
with you.  To re-enter usa, very easy.  Health cert issued within
10 days of flight and valid rabies cert.
NO quarantine, passports, or anything else at either end
