How to bring a dog out of US

Hi there,
I'm travel from France to LA,California. Got a small dog Chihuahua just  about 2 Lbs. Please let me know how can I bring her with me back to France? What paper documents do I need to be done when take her to the LAX airport? Thanks so much.

Do you have the EU pet passport? Would be a very good idea
For entry into France and EU: Need microchip   rabies vac at least 30 days old and less than
1 yr   APHIS 7001 international health cert issued within 10 days of travel and endorsed by
usda  Also, EU form 998  English-French and also endorsed by usda
