My rabbit, 12 months old, has started to lean to the left and become less active, looks a bit doped. Has free run of the garden and eats what it finds.

Your rabbit is very likely developing torticollis, which must be treated by a good rabbit vet.  Please see:

Your bunny is probably not getting adequate nutrtion from just foraging in the yard.  He needs good quality pellets, hay, and fresh greens, as described here:

He's not wild; he needs good food to survive and be healthy.  Please be sure to feed him properly, and he's much less likely to get sick (and also to recover from this illness faster).
